A Community Health Assessment (CHA) is a process by which community members gain an understanding of the health, concerns, and health care systems of the community. The process helps reveal health disparities and gaps in services among different populations (e.g., low-income, minority, or geographically isolated groups), allowing for targeted interventions that promote an equitable approach to health improvement. It is the foundation for improving the health of our community.
